Folic acid occurs naturally in the body as folate, a water-soluble vitamin also called Vitamin B9 or folacin. In this form, it is inactive and only activated in the liver by conversion to dihydrofolic acid. Folate is essential to the synthesis, repair and use of DNA. For this reason, it is crucial to rapid cell division which is an essential step in human growth. Without folic acid, the red blood cells lose their form and cannot carry their full capacity of oxygen. This quickly leads to anemia.

Indications:

MEFOL tablets are for the specific dietary management of impaired metabolic processes in those women with distinct nutritional requirements for any of the following conditions:

  • Hyperhomoeysteinemia during pregnancy,
  • High risk recurrent pregnancy loss,
  • Impaired folicacid absorption & impaired metabolism.

Folate Deficiency:

  • Folic Acid Folate deficiency can be diagnosed with a blood test. Pregnant women usually have such blood tests during prenatal checkups. Complications include Anemia and low levels of white blood cells and platelets in severe DEFICENCY cases.
  • Folic acid is especially important to the development of infants and fetuses.
  • Adults rarely show a deficiency in folic acid because it is stored in considerable amounts in the body. Still, where it presents, folic acid deficiency leads to serious health complications.
  • Milder symptoms of folic acid deficiency include diarrhea, irritability, headaches, irregular heartbeats and swollen tongue.
  • More serious signs of the deficiency are fetal defects and other complications during pregnancy, anemia, nerve damage, loss of cognitive functions, mental confusion and depression.

Folic Acid Deficiency in Pakistan:

High prevalence of folate deficiencies along with hyperhomocysteinemia in healthy Pakistani adults working in healthcare institutions is suggestive that these deficiencies could be very common in our community (general) population as well. Therefore, a serious consideration must be given towards mass micronutrient supplementation to prevent high incidence of CVD in this population. DESCRIPTION: MEFOL contains Ginger And Vitamin B6 & folate in the form of L-methylfolate which is the biologically active folate isomer, L-methylfolate is the body preferred form of folate because it is directly usable by the human organism for certain metabolic processes. There are well documented studies which have established folic acid’s ineffectiveness regarding inherited disorders of folate transport and metabolism. These disorders limit and impair the capacity to ingest, digest, absorb or metabolize folic acid. Folic acid, the synthetic form of folate, must be metabolized in a four step process by the body to become the biologically active L- methylfolate. Unmetabolized levels of folic acid were found in 78% of plasma samples from women given >400 mcg of folic acid per day. Unmetabolized folic acid has a high affinity to bind to the cellular folate transport mechanism. This has been shown to reduce the transfer of the active metabolite L-methylfolate across the blood brain barrier.

L-METHYLFOLATE SUPPLEMENTATION: The use of supplemental folacin (folic acid) may be indicated in patients with increased requirements for this vitamin. Folic acid administration may reduce the risk of neural tube defects in the developing fetus.

INFORMATION:

  • Ginger or ginger root is the rhizome of the plant Zingiber officinale. It lends its name to its genus and family (Zingiberaceae).
  • Ginger has been used throughout the world as a therapeutic agent for centuries. The herb is increasingly used in western society also, with one of the most common indications being pregnancy-induced nausea and vomiting (PNV).
  • Ginger has been suggested as a treatment for numerous other conditions, including atherosclerosis, migraine headaches .rheumatoid, rthritis .ulcers, depression , and impotence. However, there is negligible evidence for these uses.
  • Nausea and vomiting are the most common symptoms experienced in early pregnancy, with nausea affecting between 70 and 85% of women. About half of pregnant women experience vomiting.
  • Antiemetic medication appears to reduce the frequency of nausea in early pregnancy.
  • Supplementing vitamin B during pregnancy. It also contains ginger to help calm nausea and vomiting (morning sickness) during pregnancy. It may be used along with a prenatal multivitamin as determined by your doctor.

PHARMACOKINETICS: *Absorption and Elimination: L-methylfolate is a water soluble molecule which is primarily excreted via the kidneys. In a study of subjects with coromary artery disease )n-21), peak plasma levels were reached in 1 – 3 hours following ORAL/PARENTERAL administration. Peak concentrations of L-methylfolate were found to be more than seven times higher than folic acid (129 ng ml-1 vs. 14.1 ng ml- 1) following ORAL/PARENTERAL administration. The mean elimination half-life is approximately 3 hours after 5mg of oral L-methylfolate, adminstered daily for7days. The mean values for Cmax, Tmax and AUCO-12 were 129ng ml-1,1.3hours and 383 respectively.

Distribution: Red blood cells (RBCs) appear to be the storage depot for folate, as RBC levels remain elevated for periods in excess of 40 days following discontinuation of supplement. Plasma protein binding studies showed that L-methylfolate is 56% bound to plasma proteins.
